Nottingham City Signposting Service

Nottingham City Signposting Service is for people aged 60 and over who live in the Nottingham City area. The kind of help that we can provide includes:

  • finding and/or maintaining a home
  • maximising your income
  • getting the right aids and adaptations for your home, home safety and security and arranging repairs
  • coping with poor health and/or disability
  • general life skills and confidence building
  • finding social clubs or ways of meeting people
  • and much more!

We accept referrals to the Nottingham City Signposting Service directly from members of the community. You can fill in the Client Checklist yourself, or a carer or family member may fill it in for you.

You can access the service in two ways:

Self-Referral

By completing the Signposting  Client Checklist you can access the range of services listed without having to contact each of those services yourself.

Once you have completed the Checklist the Signposting Service will contact each of the services you have selected. A member of each service will then contact you to discuss your needs further. If you will need support to liaise with the services that you have selected you should tick the box for the Metropolitan Sixty Plus Support Service.

Client Checklist

Professionals working with older people can also complete the form on their client’s behalf.

The Client Checklist has a Client Information Leaflet attached. This is a combination of a receipt for the client along with information on the agencies who may visit them dependent upon their needs. Please complete the information requested and tick the appropriate boxes on the receipt and leave with your customer.

Nottingham City Signposting Service Training

We ask that professionals who complete the Client Checklist attend a half-day training session. We can also attend team meetings to provide a short training and information session if this is more convenient.
