Summerfield Close, Tamworth

Summerfield Close comprises 13 flats, where accommodation and support is provided for people aged 16-65 who have severe and enduring mental health needs and are able to live in the community with the support of a care co-ordinator.

The aim of the service is to enable customers to live independently and support is provided on a range of areas, including:

• managing a home
• health and wellbeing
• accessing leisure, employment opportunities and other services
• budgetting
• maintaining a tenancy
• benefit claims
• signposting
• general life skills
• help with resettlement when the customer is ready to move on.

Staff are on-site seven days a week from 9am-9.45pm and there is an out of hours emergency on-call service for housing related issues.
