Changing your support

We want to make sure you are getting the best support possible. If you would like to change something about your support, please just ask!

Your support worker will be able to advise you of the different options available to you.

If you would prefer not to speak to your support worker or, for example, you would like to change your support worker, please contact your local office or complete an online form.

Moving on…

At some stage, you or your support worker may decide that you are ready to receive less support or to stop receiving support from us at all.

If this happens, your support worker will work with you to make a list of all the problems that could come up if you stop getting support. You will decide together how any problems can be handled.

If you do decide to it is time to end your support, you and your support worker will fill in an exit form before you leave. You can ask us to contact you again at in the future, to check how well you have progressed.

If you decide that you need support again, you can contact us directly.
